Our Vision for 2009/10 and beyond

We will be the first choice provider of high quality sustainable and local community health care for people living in Central West London

Central West London Community Services aims to provide a wide selection of community-based services which meet the needs of all age groups and parts of our local populations. We want to establish a reputation as the provider of choice as a result of demonstrably high performance in every field in which we operate.

We aim to join up primary care, secondary care, social care and education services so that our patients/service users enjoy safe, effective and hassle-free transition through the local health and social care system, and receive the care they need in the most appropriate setting.

We will meet all our patients’ needs through our own services; and through effective partnership with other providers within and outside the NHS, wherever this enhances our service offering.

Through our highly trained and professional staff, and their in–depth local knowledge, we aim to deliver the highest standards of care in, or close to, our patients’ homes.

We support patients, parents and carers in managing their own conditions, preventing unnecessary admissions to hospital and helping patients stay at home and retain their independence for as long as possible.

We are as committed to developing health promotion, education and advocacy as we are about the prevention, treatment and cure of illness.

We aim to be a flexible and innovative organisation, which is responsive to the changing demands of patients, carers, commissioners, partners and stakeholders.

We are committed to NHS values and the pursuit of excellence in everything that we do. We seek to continuously improve our services for our patients and our clients.

We aim to base our clinical practices on the best available evidence, to be highly efficient in our use of public resources, and to be a market leader in all major areas in which we operate.
